Registered Training Practice Form – Cert II,III, IV

Thank you for offering the opportunity for an ACVN student to complete their clinical placement within your organisation. This form is about the clinic and its equipment and what will be available to students. This form covers your clinic up to ACM40418 Cert IV in Veterinary Nursing level students.

Veterinary Practice Details

Firstly, we need to see if your clinic is already registered with us and the registration is current.

If your clinic is not registered, then this form will take you through the registration process.

This list will show any clinics registered with us already and check the registration expiry date. If you clinics registration is expired, you will be directed to register the clinic first.
